You can refer into MAK if you are an adult with a mental health diagnosis.

To refer you can do this a couple of ways. One way is to go through social care- Contact your local council and request to be allocated a social worker. They will work with you to create an Adult Needs Assessment which will highlight what areas you require support in, what level of support you require.

You can also self-refer into service, to do this you can contact our office, or email our admin team. Explain that you are looking for support and you wish to self-refer into service, explain whether you require support living or if you already have your own property, leave your name and number and a manager will then get in touch with you to arrange to come and see you for a meet and greet.
